SUBCONTRACTOR FORMS AND INFORMATION:
SUBCONTRACTOR’S PRE-QUALIFICATION FORM - If you would like to be considered for Hi Tower Contractors list of subcontractors and suppliers, please e-mail your company's information to: [email protected] or fax to (718)851-3062. Please include your trade classification, at least three (3) trade references, bonding capabilities and status as a small business or minority vendor. If you do not have a standard format for providing this information, please complete the attached form.

Plans and specification in a PDF format are available for our currently active and bid projects for downloading by our Subcontractor and Vendors. If you would like access to these files, please contact our office for username and password access.

SUBCONTRACTOR FORMS AND INFORMATION:
- FORM W9 - As of January 1, 1993, federal law requires us to withhold 31% of each vendor's payment for which we do not have a correct taxpayer's identification number. Please fill out this form with your legal name (not DBA) and return to our office. Requisition payments will not be processed until this completed form is received.
- INSURANCE REQUIREMENTS
- DECLARATION OF INDEPENDENT CONTRACTOR
- REPORT OF INJURY OR ILLNESS
- JOINT CHECK AGREEMENT
- BILL OF SALE / SUBCONTRACTOR/SUPPLIER STORAGE AFFIDAVIT- Billing for stored materials, must be approved in advance and be accompanied by both a completed Bill of Sale and Storage Affidavit form. If these documents are not properly completed or received with your Requisition in a timely manner, payment may be delayed and/or your pay application may be reduced by the amount of the stored materials. In addition, you must provide proof of insurance coverage in the form of an Installation Floater that provides for the following
- COVERAGE EQUAL TO OR GREATER THAN THE AMOUNT BILLED
- LOCATION OF STORED MATERIALS
- NAME OF PROJECT
- HI TOWER CONTRACTORS INC NAMED AS CERTIFICATE HOLDER AND LOSS PAYEE BIDDING OPPORTUNITIES
